Job summary

Qualifications

• Graduate of an accredited nursing school.
• Proficient in English with strong verbal and written communication.
• Current Louisiana RN license and BLS certification.

Responsibilities

• Perform psychosocial and physiologic assessments and plan care.
• Develop and revise care plans, including patient education and discharge planning.
• Coordinate transitional health care needs and end-of-life options.
• Provide holistic care using clinical judgment and adapt education to patient/family.
• Collaborate with healthcare team to optimize patient outcomes.
• Document all patient care activities per standards.
