Job summary

Qualifications

Responsibilities

• Perform psychosocial and physiologic assessments.
• Develop and modify care plans based on patient data and trends.
• Coordinate discharge planning and patient education.
• Delegate appropriate tasks to team members while ensuring safety.
• Document all patient care activities per standards.
• Collaborate with the health care team to optimize patient outcomes.
