What You'll Do

• Support patient assessment and prioritization for walk-in and telephonic triage.
• Execute protocol-driven chronic condition management.
• Perform skilled nursing procedures including wound care and medication administration.
• Provide comprehensive patient and caregiver education regarding chronic conditions and preventative care.
• Conduct medication reviews within nurse-led visits.
• Conduct onsite testing and use patient data to identify gaps in preventative screenings.
• Review and act upon high-priority abnormal laboratory results.
• Partner with providers to manage complex care plans and coordinate transitions from hospital to clinic.
• Utilize standardized protocols for medication management and prescription renewals.
• Delegate tasks to medical assistants and phlebotomists as needed.

Requirements

Education:

• Active, Unrestricted Registered Nurse Licensure in the state that you reside

Experience:

• 2+ years of healthcare experience, working as an RN preferred

Licenses & Certifications:

• BLS or CPR Certification required
• Electronic Medical Record (EMR) experience required
