Job summary

CVS Health in High Point, NC is seeking a Registered Nurse to deliver quality patient care, coordinate services, and support chronic disease management for diverse patient populations.

You will triage and educate patients, collaborate with the care team, and follow standardized protocols for medications, refills, and prior authorizations to ensure a seamless, safe care experience. Additionally, you will participate in daily huddles and care plan reviews to monitor progress and outcomes.

Qualifications

• Active North Carolina RN licensure in good standing.
• Hands-on patient care experience in a clinic setting.
• CPR/BLS certification and EMR experience.

Responsibilities

• Provide competent nursing care by executing job responsibilities safely and consistently.
• Respond to incoming telephonic requests, ensuring reliability and responsiveness to patient needs.
• Provide clinically competent triage and symptom management to patients.
• Utilize standardized protocols for medication management, prescription refills, and prior authorizations.
• Conduct thorough and accurate reviews of patient medications and update as needed.
• Provide comprehensive education and direct patient care, particularly around chronic conditions.
• Actively collaborate and monitor the implementation and progress of care plans for patients.
• Delegate activities to other clinical team members to support patient needs.
• Form relationships with patients and caregivers to support preventative care.
• Participate in care team meetings and daily huddles to discuss patient care and clinic operations.

Requirements

Education:

• Active Registered Nurse (RN) Licensure in good standing with the state of North Carolina

Experience:

• Previous experience in a clinic setting with hands-on patient experience
• 2+ years of healthcare experience preferred

Licenses & Certifications:

• Active CPR and/or BLS Certification required
• Electronic Medical Record (EMR) experience required
