Job summary

Palms Home Care is seeking Registered Nurses (RNs) to join our home health team in Florida. We offer full-time and PRN positions, with opportunities to work across diverse patient cases and collaborate with an integrated care team.

In this role, you will perform patient assessments, develop care plans, and educate patients and caregivers to promote independence. Travel between patient homes is required, with support from LPNs and the care team.

Qualifications

• Current Florida RN license required.
• Minimum 1 year home health nursing experience.
• Knowledge of nursing care procedures and safety guidelines.
• Ability to work independently and manage daily schedule.
• Strong communication and interpersonal skills.
• Familiarity with Kinnser EHR.
• Willingness to travel between patient homes.

Responsibilities

Responsibilities

• Perform comprehensive patient assessments and develop individualized care plans in collaboration with the care team.
• Complete evaluation tasks, including reviewing medication and vital signs.
• Coordinate care by making appropriate referrals or documenting clinical rationale when referrals are not indicated.
• Communicate effectively with the clinical manager and interdisciplinary team regarding patient status and care updates.
• Educate patients and caregivers to promote independence and improve health outcomes, including training in self-care techniques.
• Coordinate with Physicians, Physical Therapists, Occupational Therapists and other individuals in the patient’s care plan.
• Supervise and support LPNs to ensure high standards of care.
• Perform additional duties as needed to support patients and team operations.

Qualifications

• Current Registered Nurse (RN) license in the state of Florida
• Must have at least 1 year experience as a home health nurse
• Excellent knowledge of nursing care methods and procedures
• In-depth knowledge of health and safety guidelines and procedures
• A team player with excellent communication and interpersonal skills
• Outstanding organizational and multi-tasking skills
• Patient with excellent problem-solving skills
• Familiar with EHR such as Kinnser.
• Ability to work independently, manage a daily schedule, and travel between patient homes
