Job summary

Nest & Care in the United States seeks a dedicated and compassionate PRN Registered Nurse (RN) to join our home care team. You will provide essential nursing care to patients in their homes, with a flexible schedule and meaningful impact on families.

The role requires an MD state RN license with Trach care experience, CPR/First Aid, and ability to drive to cases. Prior home care experience is preferred; EMR familiarity is advantageous.

Qualifications

• RN license in MD with Trach care experience is required.
• Current CPR and First Aid certification.
• TB vaccination and Hep B vaccination.
• Graduation from an accredited nursing program (diploma/transcript).
• Clear criminal background and driving record.
• Two professional healthcare references.
• Prior home care experience strongly preferred.
• Self-starter with ability to work independently and manage time.
• Strong interpersonal skills and ability to mentor caregivers.
• Comfort with technology; EMR experience preferred.
• Ability to drive and travel to cases as assigned.

Responsibilities

• Meets new and prospective clients and performs initial assessment of service needs (virtual or in-person).
• Develops a Plan of Care for all new clients and updates as needed.
• Communicates with care managers, caregivers, office staff, clients, family members and medical professionals about client needs.
• Consults with Clinical Director regarding supervision of services.
• Provides training to clients regarding their care when requested.
• Maintains clinical records in compliance with state and federal laws.
• Performs client evaluations every 30, 90 or 120 days as needed by the Plan of Care.
• Performs caregiver evaluations every 30, 90 or 120 days as needed by state requirements.
• Provides skills evaluations for new staff during orientation.
• Trains new staff and existing staff.
• Provides Clinical Oversight.
