Job summary

Elara Caring is seeking a Registered Nurse Home Health to join the team and deliver right-time, right-place care in patients' homes across Texas. You will assess patients, perform ongoing evaluations, and educate families while maintaining HIPAA privacy and patient safety.

Requirements include an active RN license in the state, 1+ year of clinical nursing experience, and the ability to travel between patient homes using a vehicle. Home health experience is preferred but not required.

Qualifications

• Current State license as a Registered Nurse.
• Graduate of an accredited Nursing program (Certificate, Diploma, ADN, or BSN).
• 1+ year of nursing experience in a clinical setting.

Responsibilities

• Assessing patients to determine their care needs.
• Completing initial and ongoing patient assessments.
• Protecting patient and staff privacy per HIPAA.
• Providing nursing care according to the plan of care and physician orders.
• Monitoring patient conditions and reporting changes to the care team.

As a Registered Nurse Home Health ,you’llcontribute to our success in the following ways:

What is Required?

• Current State license as a Registered Nurse.
• Graduate of an accredited Certificate, Diploma, Associates or Baccalaureate School of Nursing.
• 1+ year of experience as a nurse in a clinical care setting.
• Home Health experience is preferred, but not required.
• Ability to frequently lift, push, pull, and support up to 50 pounds, including positioning or transferring patients and moving equipment
• Must possess a valid driver’s license, maintain an acceptable driving record, OR have reliable transportation to travel between patient homes and assigned work locations.
