Job summary

Tennessee Quality Care is seeking a Registered Nurse (RN) for a full-time Home Health role in Wayne, Lawrence, and Hardin Counties. You will provide skilled nursing care in patient homes, guiding recovery and independence with a patient-centered approach.

Qualifications

• Active RN license and nursing degree.
• CPR certification required.
• Minimum 1 year of clinical experience (ICU, Med-Surg, or Rehab preferred).

Responsibilities

• Perform patient assessments, admissions, and ongoing care planning.
• Coordinate care with physicians and interdisciplinary teams.
• Deliver therapeutic, preventative, and rehabilitative nursing care.
• Monitor patient progress and update plans of care.
• Support patients and families through all stages of care.
• Maintain accurate and timely documentation.

What You’ll Do
