Job summary

Tennessee Quality Care in Nashville seeks a compassionate Registered Nurse (RN) to join our Home Health team on a full-time basis. Provide skilled nursing care directly in patients' homes, assess needs, plan care, and coordinate with physicians.

You’ll enjoy a flexible schedule, mileage reimbursement, 401(k) match, and a supportive culture focused on work-life balance. Applicants should have an active RN license, at least 1 year of clinical experience, CPR, and home health experience is a plus.

Qualifications

• Active RN license in Tennessee.
• Graduate of an accredited nursing program.
• Minimum 1 year of clinical experience (ICU, Med-Surg, or Rehab preferred).
• Home Health experience is a plus.
• Current CPR certification.

Responsibilities

• Performing patient assessments, admissions, and ongoing care planning.
• Coordinating care with physicians and interdisciplinary teams.
• Delivering therapeutic, preventative, and rehabilitative nursing care.
• Monitoring patient progress and updating plans of care.
• Supporting patients and families through all stages of care.
• Maintaining accurate and timely documentation.
