Qualifications

Responsibilities

• Assess patients: Evaluate physical, emotional, spiritual, and educational needs with the healthcare team.
• Develop care plans with patients and families to prioritize needs and goals.
• Deliver patient care: Provide evidence-based care, educate patients and families, and implement procedures per policies.
• Evaluate outcomes: Monitor patient responses and adjust care based on outcomes and evidence.
• Drive improvement: Collaborate with Clinical Manager on Performance Improvement initiatives.
• Promote High Reliability: Apply High Reliability principles to improve care and efficiency.
• Lead and mentor: Motivate, educate, and mentor peers while managing patient care.
• Document care: Accurately document nursing process, plan of care, and responses in EMR.
• Maintain compliance: Follow policies and complete required training.
