Job summary

Loving Care In-Home Health Services in Shawnee, OK is seeking an experienced RN to supervise and manage patient care for home-health cases. You will coordinate nursing staff, home-health aides, social work and therapies, ensuring orders are followed and assessments are completed.

Responsibilities

• 1. Supervise and manage all care given to assigned case management patients. Responsible for identifying patient and/or family needs and overseeing all patient care management for assigned patients, to include nursing staff, homehealth aides, social work and all therapies that are assigned to patients as ordered by physician.
• 2. Responsible for providing supportive care and appropriate services in accordance with attending physician's orders and the interdisciplinary team plan of care.
• 3. Coordinate work schedule to fit patient's time and needs. Spend minimum 30 minutes to 1 hour in length in each home to do total head-to-toe assessments, vital signs, administer medications as ordered, complete supervisory of patient care assessments and give treatments as ordered by physician.
• 4. All patient visit documentation must be completed and submitted electronically no later than 24 hours of the time patients visits are completed.
• 5. Develop Plan of Care with all orders and communication going to patient's physician.
• 6. PRN visits will be made as they occur and out of necessity.

Qualifications

• Must have current Registered Nursing license approved by the Oklahoma Nursing Board of Regents without restrictions.
• At least one year experience in either home health setting or acute-care setting with experience in elderly, middle aged, and/or pediatric care.
• Must be able to lift a minimum of 20 lbs without difficulty.
• Must own a vehicle in which to make home visits and maintain current automobile insurance coverage.
• Must have a telephone available for after-hours calls when needed.
