Job summary

Palms-Home-Care in Florida is expanding and seeks dedicated Home Health RNs for full-time and PRN roles. You will perform comprehensive patient assessments, develop care plans, and coordinate with the care team to deliver compassionate in-home nursing.

The ideal candidate holds an active Florida RN license, has at least 1 year of home health experience, is proficient with EHR systems such as Kinnser, and can travel between patient homes while maintaining excellent communication with the clinical

Qualifications

• Current Florida RN license (active).
• At least 1 year of home health nursing experience.
• Excellent nursing knowledge and patient care skills.
• Knowledge of health and safety guidelines and procedures.
• Strong communication and interpersonal abilities.
• Ability to work independently and manage a daily schedule.

Responsibilities

• Review medications and vital signs and complete evaluation tasks.
• Coordinate care through referrals or documented clinical rationale when referrals are not indicated.

• Coordinate with physicians, therapists, and other team members involved in the patient’s plan of care.
• Supervise and support LPNs to maintain high standards of care.
• Perform additional duties to support patients and team operations.

• Complete evaluation tasks, including reviewing medication and vital signs.

• Communicate effectively with the clinical manager and interdisciplinary team regarding patient status and care updates.
• Educate patients and caregivers to promote independence and improve health outcomes, including training in self-care techniques.
• Coordinate with Physicians, Physical Therapists, Occupational Therapists and other individuals in the patient’s care plan.
• Supervise and support LPNs to ensure high standards of care.

Qualifications

• Current Registered Nurse (RN) license in the state of Florida
• Must have at least 1 year experience as a home health nurse
• Excellent knowledge of nursing care methods and procedures
• In-depth knowledge of health and safety guidelines and procedures
• A team player with excellent communication and interpersonal skills
• Outstanding organizational and multi-tasking skills
• Patient with excellent problem-solving skills
• Familiar with EHR such as Kinnser.
• Ability to work independently, manage a daily schedule, and travel between patient homes
