Job summary

Moses Lake Community Health Center seeks a registered nurse to lead advanced care coordination for complex patients and provide direct care in the clinic. The role includes serving as a clinical resource to both patients and staff and acting as the primary clinical liaison for Advanced Care Coordination.

The nurse will ensure accurate medical chart updates and follow quality, safety, and infection control procedures while supporting the care team and patients.

Qualifications

  • RN licensure in the state of Washington is required.
  • Associate degree or diploma in Nursing from an accredited program.
  • BLS certification is required.
  • Bilingual/ multilingual abilities are preferred but not required.

Responsibilities

  • Lead advanced care coordination for complex patients.
  • Provide direct patient care in the clinic.
  • Serve as a clinical resource to patients and staff.
  • Act as primary clinical liaison for Advanced Care Coordination.

Brief Description

They also serve as a clinical resource to both patients and the medical team, providing direct patient care in the clinic and as a resource for triage and questions by phone.

They also serve as a clinical resource to both patients and the medical team, providing direct patient care in the clinic and as a resource for triage and questions by phone.

Responsibilities
  • Leads the care coordination an treatment for complex patient care.
  • Serves as a clinical resource to both patients and internal staff.
  • Serves as the primary clinical communication liaison for Advanced Care Coordination.
  • Ensures that the patient's medical chart is updated to reflect treatments and care provided.
  • Performs other direct patient care duties within a scope of practice.
  • Properly follows quality, safety, and infection control procedures and improvement efforts.
  • Provides excellent service and support to patients and colleagues and as a leader within the team.
Requirements
  • Job duties require knowledge and training in the field of Nursing typically acquired through a formal Associates degree or trade school program in Nursing.
  • Must have active Registered Nursing licensure with the State of Washington.
  • Must have obtained Basic Life Support (BLS) certification.
  • Ability to read and speak Spanish and/or Russian preferred but not required.
  • Must have reliable transportation and provide documentation of auto insurance and valid driver's license as required.
  • Travel between work sites is required as needed.
