Job summary

Legacy Hospice, Inc. in Poplar Bluff, MO is seeking a compassionate Registered Nurse Case Manager (RNCM) to deliver high-quality end-of-life care in patients' homes. You will coordinate care with the interdisciplinary team, educate families, manage a caseload, and ensure accurate documentation while adhering to safety and compliance standards.

You will also participate in on-call rotations, supervise staff when needed, and advocate for patients and families throughout the care continuum.

Qualifications

• RN license in Missouri (or eligible)
• BSN or CHPN preferred
• Minimum 1 year nursing experience in hospice, home health, or related field
• Strong interpersonal, clinical, and organizational skills
• Active CPR/BLS certification
• Valid driver’s license and reliable transportation
• Ability to work independently and with a team
• Pre-employment drug screening required (drug-free workplace)

Responsibilities

• Perform comprehensive patient assessments and create individualized care plans
• Manage a caseload of hospice patients and coordinate care with the IDT
• Administer medications and treatments per physician orders
• Educate patients and families on symptom management and end-of-life care
• Monitor and update care plans based on ongoing assessments
• Provide emotional and clinical support to patients and caregivers
• Communicate with physicians, care partners, and community resources
• Maintain accurate, timely clinical documentation
• Participate in IDT meetings and on-call rotation
• Supervise ancillary staff and ensure continuity of care

• Active CPR/BLS certification
• Valid driver’s license, auto insurance, and reliable transportation
• Ability to work independently and with a team
