Job summary

Legacy Hospice, Inc. is seeking a Registered Nurse Case Manager (RNCM) to deliver high-quality end-of-life care in patients' homes from Harrison, AR. You will coordinate interdisciplinary care and support families with empathy and professionalism.

The role requires an active RN license in Arkansas, with BSN or CHPN preferred and at least 1 year in hospice/home health. This salaried, exempt position includes mileage reimbursement, PTO, and continuing education opportunities.

Qualifications

• RN licensed in the state of employment
• BSN or CHPN preferred
• Minimum 1 year of hospice/home health nursing experience
• Strong interpersonal, clinical, and organizational skills
• Active CPR/BLS certification
• Valid driver’s license and transportation

Responsibilities

• Perform comprehensive patient assessments and create individualized care plans
• Manage a caseload of hospice patients and coordinate care with the IDT
• Administer medications and treatments per physician orders
• Educate patients and families on symptom management and end-of-life care
• Monitor and update care plans based on ongoing assessments
• Provide emotional and clinical support to patients and caregivers
• Communicate with physicians, care partners, and community resources
• Maintain accurate, timely clinical documentation
• Participate in IDT meetings and on-call rotation
• Supervise ancillary staff and ensure continuity of care
