Job summary

Legacy Hospice, Inc. in Leawood, Kansas seeks an experienced Registered Nurse Case Manager (RNCM) to deliver compassionate, home-based end-of-life care and support families with empathy and respect.

This is a salaried, full-time role with opportunities for continuing education and professional growth. You will work with an interdisciplinary team to assess patients, develop care plans, administer medications, educate families, and maintain thorough documentation while participating in on-call

Qualifications

• Registered Nurse (RN) license in the state of employment.
• BSN or CHPN preferred.
• Minimum 1 year of nursing experience in hospice, home health, or related field.
• Active CPR/BLS certification.
• Valid driver’s license, auto insurance, and reliable transportation.
• Ability to work independently and with a team.
• Drug-free workplace; pre-employment drug screening including marijuana.

Responsibilities

• Perform comprehensive patient assessments and create individualized care plans.
• Manage a caseload of hospice patients, coordinating care with the interdisciplinary team.
• Administer medications and treatments per physician orders.
• Educate patients and families on symptom management, medication, and end-of-life care.
• Monitor and update care plans based on ongoing patient assessments.
• Provide emotional and clinical support to patients and caregivers.
• Communicate with physicians, care partners, and community resources.
• Maintain accurate, timely clinical documentation.
• Participate in interdisciplinary team (IDT) meetings and on-call rotation.
• Supervise ancillary staff and ensure continuity of care.
