Angels Care Home Health is seeking a Registered Nurse (RN) Case Manager to coordinate patient home care and collaborate with medical teams in Pearland, Texas.

You will perform admission visits, determine eligibility for home care services, and develop the plan of care in collaboration with the patient and family, while providing professional nursing care and documenting activities. This role supports professional growth and offers a comprehensive benefits package including training and ample PTO.

Formación

• Active RN license in the state where services are provided.
• CPR certification current.
• Valid driver''s license and reliable transportation.
• 1 year RN experience preferred; 2+ years preferred.

Responsabilidades

• Coordinate all aspects of the patient''s home care and communicate with medical teams.
• Make admission visits to patients in their homes and assess eligibility for home care.
• Develop and implement the plan of care in collaboration with the patient and family.
• Provide professional nursing care and document care activities per policy.

About the position:

The Registered Nurse (RN) Case Manager coordinates all aspects of the patient’s home care. You will interact with other medical disciplines as needed & provide professional care to the patient. You will also perform the admission visit to the patient in the home, determine the patient’s eligibility for home care services, & develop the plan of care to be followed.

Qualifications:

• Graduate of an accredited diploma, Associate or Baccalaureate School of Nursing.
• Current state license as a Registered Nurse.
• Current state Driver’s License.
• One-year's experience as a Registered Nurse, two preferred.
• Proof of current CPR and Hepatitis consent/declination.
• Reliable transportation with valid and current auto liability insurance.
