Angels Care Home Health is seeking an experienced Registered Nurse (RN) Case Manager to coordinate all aspects of a patient’s home care. You will interact with multiple medical disciplines, provide professional nursing care, and conduct the admission visit in the patient’s home to determine eligibility and develop the plan of care.

Requirements include RN license in the state, valid driver’s license, CPR, and reliable transportation. One year of RN experience is preferred.

Formación

• Graduate of an accredited diploma, Associate or Baccalaureate School of Nursing.
• Current state RN license.
• Valid driver’s license.
• Minimum one year RN experience; two years preferred.
• CPR certification and hepatitis consent/declination.
• Reliable transportation with current auto liability insurance.

Responsabilidades

• Coordinate all aspects of the patient’s home care.
• Interact with other medical disciplines as needed.
• Perform the admission visit to the patient in the home.
• Determine patient eligibility for home care services and develop the plan of care.

About the position:

The Registered Nurse (RN) Case Manager coordinates all aspects of the patient’s home care. You will interact with other medical disciplines as needed & provide professional care to the patient. You will also perform the admission visit to the patient in the home, determine the patient’s eligibility for home care services, & develop the plan of care to be followed.
