Job summary

Heartland Hospice, part of ProMedica, seeks a compassionate Registered Nurse On Call to provide after-hours hospice care in home and facility settings. This role responds to urgent patient needs, triages calls, conducts visits, manages symptoms, and provides education and emotional support to patients and families.

The RN collaborates with an interdisciplinary team to implement individualized care plans focused on comfort, dignity, and quality of life.

Qualifications

• Experience providing hospice or palliative care in home or facility settings.
• Strong communication and empathetic patient/family support.
• Valid RN license with current state requirements.

Responsibilities

• Triage calls and respond to urgent patient needs after hours.
• Conduct visits and manage symptoms for comfort and quality of life.
• Provide education and emotional support to patients and families.
• Collaborate with interdisciplinary team to implement care plans.
• Perform comprehensive assessments and medication/pain management.
• Document care activities and coordinate with on-call providers.
• Ensure seamless transitions of care in a mission-driven environment.
