Heartland Hospice is seeking a compassionate Registered Nurse On Call to provide after-hours hospice care to patients with life-limiting illness in homes and facilities.

The RN triages calls, conducts urgent visits, manages symptoms, and coordinates care with the interdisciplinary team, physicians, and caregivers to support comfort and end-of-life goals.

Responsibilities

• Triage calls and determine urgency for hospice patients.
• Perform urgent in-home or facility visits as needed.
• Assess and manage pain and symptoms to improve comfort.
• Coordinate care with the interdisciplinary team and physicians.
• Educate patients and families, document care, and ensure smooth transitions.
