Palms Home Care is seeking a dedicated Registered Nurse (RN) for home health in Orlando, FL. Responsibilities include patient assessment, care planning, and coordinating with physicians and therapists to support patient independence.

The role requires Florida RN license, at least 1 year in home health, strong communication, organization, and ability to travel to patient homes. Travel and caregiving support are essential to deliver quality care.

Qualifications

Job description

Reports To: Clinical Manager

Responsibilities

• Perform comprehensive patient assessments and develop individualized care plans in collaboration with the care team.
• Complete evaluation tasks, including reviewing medication and vital signs.
• Coordinate care by making appropriate referrals or documenting clinical rationale when referrals are not indicated.
• Communicate effectively with the clinical manager and interdisciplinary team regarding patient status and care updates.
• Educate patients and caregivers to promote independence and improve health outcomes, including training in self-care techniques.
• Coordinate with Physicians, Physical Therapists, Occupational Therapists and other individuals in the patient’s care plan.
• Supervise and support LPNs to ensure high standards of care.
• Perform additional duties as needed to support patients and team operations.

Qualifications

• Must have at least 1 year experience as a home health nurse
• Excellent knowledge of nursing care methods and procedures
