Job summary

The Palms Home Care in Leesburg, FL is seeking dedicated Home Health RNs for full-time and PRN positions. You will assess patients, develop care plans, coordinate with the care team, and educate patients and caregivers to promote independence.

Qualifications

• Current Registered Nurse (RN) license in Florida
• Minimum 1 year experience as a home health nurse
• Knowledge of nursing care methods and procedures
• Familiar with health and safety guidelines and procedures
• Excellent communication and interpersonal skills
• Strong organizational and multitasking abilities
• Problem-solving skills
• Ability to work independently, manage daily schedule, and travel to patient homes
• Experience with EHR systems (Kinnser)

Responsibilities

• Perform comprehensive patient assessments and develop individualized care plans with the care team
• Review medications and vital signs as part of evaluations
• Coordinate care via referrals or documenting clinical rationale when referrals are not indicated
• Communicate with clinical manager and interdisciplinary team about patient status
• Educate patients and caregivers to promote independence and self-care
• Coordinate with physicians, physical and occupational therapists and other care team members
• Supervise and support LPNs to maintain high standards of care
• Perform additional duties to support patients and team operations

Responsibilities

• Perform comprehensive patient assessments and develop individualized care plans in collaboration with the care team.
• Complete evaluation tasks, including reviewing medication and vital signs.
• Coordinate care by making appropriate referrals or documenting clinical rationale when referrals are not indicated.
• Communicate effectively with the clinical manager and interdisciplinary team regarding patient status and care updates.
• Educate patients and caregivers to promote independence and improve health outcomes, including training in self-care techniques.
• Coordinate with Physicians, Physical Therapists, Occupational Therapists and other individuals in the patient’s care plan.
• Supervise and support LPNs to ensure high standards of care.
• Perform additional duties as needed to support patients and team operations.

Qualifications
