Job summary

Palms Home Care is seeking a dedicated Home Health RN (FT and PRN) in Florida to join our expanding team. You will perform comprehensive patient assessments, develop individualized care plans, and coordinate with a multidisciplinary care team to ensure excellent patient outcomes.

The role requires an active FL RN license, at least 1 year in home health, and willingness to travel between patient homes while educating patients and caregivers for independence.

Qualifications

Responsibilities

Responsibilities

• Perform comprehensive patient assessments and develop individualized care plans in collaboration with the care team.
• Complete evaluation tasks, including reviewing medication and vital signs.
• Coordinate care by making appropriate referrals or documenting clinical rationale when referrals are not indicated.
• Communicate effectively with the clinical manager and interdisciplinary team regarding patient status and care updates.
• Educate patients and caregivers to promote independence and improve health outcomes, including training in self-care techniques.
• Coordinate with Physicians, Physical Therapists, Occupational Therapists and other individuals in the patient’s care plan.
• Supervise and support LPNs to ensure high standards of care.
• Perform additional duties as needed to support patients and team operations.

Qualifications

• Current Registered Nurse (RN) license in the state of Florida
• Must have at least 1 year experience as a home health nurse
• Excellent knowledge of nursing care methods and procedures
• In-depth knowledge of health and safety guidelines and procedures
• A team player with excellent communication and interpersonal skills
• Outstanding organizational and multi-tasking skills
• Patient with excellent problem-solving skills
• Familiar with EHR such as Kinnser.
• Ability to work independently, manage a daily schedule, and travel between patient homes
