Berkeley Health and Rehab in Berkeley, IL is seeking a compassionate Registered Nurse (RN) to join our close-knit nursing team. You will lead and provide direct resident care, oversee care plans, and coordinate with physicians and families to ensure outstanding service.

Qualifications

• Current Illinois RN license required.
• Experience in skilled nursing or long-term care preferred.
• Strong clinical, organizational, and leadership skills.

Responsibilities

• Provide and oversee nursing care in accordance with physician orders, resident care plans, and facility policies.
• Conduct nursing assessments and monitor residents for changes in condition.
• Administer medications and treatments as prescribed.
• Coordinate resident care with physicians, families, and other healthcare professionals.
• Develop, implement, and update individualized resident care plans.
• Supervise, guide, and support LPNs and CNAs.
• Respond to changes in residents' conditions and provide appropriate nursing interventions.
• Ensure accurate and timely clinical documentation.
• Promote residents' dignity, comfort, safety, and overall well-being.
• Maintain compliance with infection control standards and applicable regulations.

Responsibilities

• AM Shift: 7:00 AM – 3:00 PM

• PM Shift: 3:00 PM – 11:00 PM

• NOC Shift: 11:00 PM – 7:00 AM

• Rotating weekends required
