Job summary

Penn-Mar Human Services is seeking a Registered Nurse (RN) to oversee and coordinate medical care for people supported, including attending appointments and hospital visits, and training team members on new procedures.

The role requires PA RN licensure, a valid driver's license, and high school diploma with preferred 1 year nursing experience in healthcare. On-site schedule is Mon-Fri with occasional after-hours duties and possible after-hours visits.

Qualifications

• RN license or compact licensure suitable for PA; PA RN license required.
• High School Diploma or GED is required.
• One year of nursing experience in a healthcare setting is preferred.

Responsibilities

• Oversee and coordinate medical care for people supported, including accompanying to appointments and hospital visits, supporting discharges, and training team members on new procedures.
• Assist with medication management and coordinate with pharmacies.
• Liaise with medical providers and team members to implement recommendations.
• Attend HRST Clinical Reviewer Certification and complete timely clinical reviews.

Job description

• Stable Mon-Fri work schedule 8:00a-4:30p, primarily in an office setting
• Minimal On-Call Responsibility: available to respond to after-hours health-related inquiries from direct support team members via phone call or text message, providing clinical direction as needed. In certain situations, an occasional evening or weekend on-site visit may be required to address urgent health or support needs.

The Registered Nurse (RN) is responsible for:

• Oversight and coordination of medical care for people supported, including:
  • attending medical appointments and hospital visits, supporting hospital/rehab discharges, and train team members on new procedures
  • actively participating in hospital discharges, ensuring team members are trained on any changes or new procedures
  • obtaining medical equipment and supplies
  • coordinating equipment inspections and repairs as needed
  • assisting with medication management and coordinating with pharmacies
  • liaising with medical providers and team members, other support staff
  • reviewing medical documentation for accuracy and ensuring recommendations are implemented
• HRST
  • obtain HRST Clinical Reviewer Certification
  • complete timely Clinical Reviews
  • review and apply training recommendations in coordination with operations supervisors
• Training
  • manage PA Medication Administration initial training and ensure timely revalidation for all PA team members
  • support Penn-Mar's Learning & Development team by facilitating organizational training as needed
  • Provide thorough training in tasks including feeding tube procedures, vital signs, nebulizer treatments, lifts, oxygen therapy, specialized diets, etc.
• Responding to after-hours calls and messages from direct support team members, providing clinical direction as needed
• Following all incident reporting based on state requirements in a timely manner
• Actively participating in ODP or PAR nursing related meetings/groups
• Hold a Pennsylvania RN license
• Valid driver's license with at least 2 years of driving experience on a full license (time spent driving on a learner's permit/provisional license does NOT count toward this requirement)
• High School Diploma/GED
• One year of nursing experience in a healthcare setting preferred
• Previous experience supporting individuals with disabilities preferred

Physical Requirements:

• Lift and carry or otherwise move items weighing 20/50 pounds regularly/occasionally
• Transfer or assist in transferring individuals
• Adequate visual acuity to perform essential functions
• Read, write, and speak English to communicate with individuals and team members
