Job summary

Qualifications

  • PA RN license required.

Responsibilities

  • Coordinate and oversee medical care for people supported, including attending medical appointments and hospital visits.
  • Support hospital/rehab discharges and train team on new procedures.
  • Assist with medication management and coordinate with pharmacies.
  • Liaise with medical providers and team members.
  • Review medical documentation for accuracy and implement recommendations.
  • Obtain HRST Clinical Reviewer Certification and complete timely reviews.
  • Coordinate PA Medication Administration training and revalidation for team members.
  • Respond to after-hours calls and provide clinical direction as needed.
  • Attend nursing-related meetings/groups.

Job description

  • Minimal On-Call Responsibility: available to respond to after-hours health-related inquiries from direct support team members via phone call or text message, providing clinical direction as needed. In certain situations, an occasional evening or weekend on-site visit may be required to address urgent health or support needs.

The Registered Nurse (RN) is responsible for:
  • Oversight and coordination of medical care for people supported, including:
    • attending medical appointments and hospital visits, supporting hospital/rehab discharges, and train team members on new procedures
    • actively participating in hospital discharges, ensuring team members are trained on any changes or new procedures
    • obtaining medical equipment and supplies
    • coordinating equipment inspections and repairs as needed
    • assisting with medication management and coordinating with pharmacies
    • liaising with medical providers and team members, other support staff
    • reviewing medical documentation for accuracy and ensuring recommendations are implemented
  • HRST
    • obtain HRST Clinical Reviewer Certification
    • complete timely Clinical Reviews
    • review and apply training recommendations in coordination with operations supervisors
  • Training
    • manage PA Medication Administration initial training and ensure timely revalidation for all PA team members
    • support Penn-Mar's Learning & Development team by facilitating organizational training as needed
    • Provide thorough training in tasks including feeding tube procedures, vital signs, nebulizer treatments, lifts, oxygen therapy, specialized diets, etc.
  • Responding to after-hours calls and messages from direct support team members, providing clinical direction as needed
  • Following all incident reporting based on state requirements in a timely manner
  • Actively participating in ODP or PAR nursing related meetings/groups
  • Hold a Pennsylvania RN license
  • Valid driver's license with at least 2 years of driving experience on a full license (time spent driving on a learner's permit/provisional license does NOT count toward this requirement)
  • High School Diploma/GED
  • One year of nursing experience in a healthcare setting preferred
  • Previous experience supporting individuals with disabilities preferred
Physical Requirements:
  • Lift and carry or otherwise move items weighing 20/50 pounds regularly/occasionally
  • Transfer or assist in transferring individuals
  • Adequate visual acuity to perform essential functions
  • Read, write, and speak English to communicate with individuals and team members
