Job summary

County of Chester is seeking a Registered Nurse to provide direct patient care in a skilled nursing setting. The RN will assess changes in condition, document findings, and supervise CNAs to ensure smooth shift transitions.

Qualifications

Responsibilities

Job description

The Registered Nurse performs and provides prescribed medical treatment and personal care to residents in a skilled nursing setting. The RN assesses all resident change in condition and promptly documents the results. Assists the Unit Manager and Supervisor with ensuring continuity of care between the shifts. The RN is also responsible for supervising Certified Nursing Assistants and other supportive staff members working on the nursing unit. Maintains role as resident advocate with a focus on the facility’s mission.

Responsibilities
  • Adequately assess residents for pain and administer medications as ordered
  • Performs all aspects of resident care in an environment that optimizes resident safety
  • Collect samples, such as urine and sputum. Also completes glucose testing on residents as ordered
  • Completes written documentation and assessments in a timely, clear, and concise manner
  • Complete treatments as ordered
  • Assemble and use equipment such as catheters, tracheotomy tubes, and oxygen suppliers.
  • Notes and carry’s out physician orders in a timely manner
  • Administer medications according standards set forth in the nurse practice act including but not limited to oral, intramuscular, or subcutaneous
  • Monitor for adverse effects of medication and makes physician aware
  • Communicate residents’ status and/or needs to the supervisor physician, consultants and Nursing Assistants.
  • Supervise activities by directing, communicating and monitoring Certified Nurse Assistants (CNA’s).
  • Assess change in resident condition including but not limited to falls, acute illness, or mental status changes.
  • Completes a full head to toe assessment on all new admissions and hospital returns
  • Implements plan of care for the resident based on assessments and goals as established by the interdisciplinary care team
  • Treats the residents and their families with dignity and respect
  • Suports and maintains a culture of safety and quality
  • Gives a thorough report to oncoming shift by participating in walking rounds
  • Works closely with the Lead Nursing Assistant to maintain optimal workflow and over all functioning of the unit.
  • Perform other duties, tasks and special projects, as required.
Qualifications
  • Associate’s degree from an accredited college or university.
  • An unrestricted RN license.
  • Current Certification in CPR.
  • Six months to one year of job-related experience and/or training.
  • Mandated in-services to meet state regulations.
  • Educational and clinical experience required by State Nursing Board.
  • Ability to establish priorities and remain focused on daily operations.
  • Effective interpersonal and customer service skills.
  • Strong organizational, time management, and documentation skills.
  • Accurate and detail oriented with strong oral and written communication skills.
  • Ability to work independently or as part of a team and proceed with objectives.
  • Ability to follow protocols to handle and resolve recurring problems.
  • Ability to use common sense understanding to carry out written and verbal instructions.
  • Flexible, with an ability to carry multiple tasks and projects to completion
  • Exhibits a strong work ethic and a willingness to learn new tasks.
  • Professionalism, integrity, and honesty with an ability to handle confidential matters.
Preferred Skills, Knowledge & Experience:
  • One or more years of job related expereience.
  • Attend in-services and seminars related to geriatric population
  • Strong knowledge of Pennsylvania state regulations.
  • Ability to complete continuing education and remain updated on the health care field via classes, seminars, magazines, journals, literature and videos.
  • Working knowledge of Pocopson Home policies and procedures.Working knowledge of Nursing Board regulations.
Relevant background checks, clearances, physicals, drug tests, and any other assessments or evaluations will be conducted after an offer of employment is given per County policy.
Computer Skills:

General knowledge of computers with ability to navigate electronic documentation and administer medications

Physical Demands:

While performing the duties of this position, the employee is frequently required to stand; walk; reach above shoulders and work with arms above shoulder height; bend at the waist or work bent at the waist; climb stairs; push wheelchairs or other items; lift patients and items; work with machinery (e.g., feeding pumps, oxygen tanks); talk or hear; and taste or smell. Occasionally, the employee will be required to sit; kneel, stoop, crouch or squat; lift and carry items; and work with an uncommon level of noise. On rare occasions, you will need to crawl, and twist or rotate at the waist.

Work Environment:
  • The noise level varies from quiet to moderately noisy.
  • Will work inside 100% of the time.

To continue to be employed in a part-time position, you are expected to meet the following scheduling requirements:

  • Minimum Weekly Shift: Must work at least one shift per week.
  • Weekend Coverage: Must work at least two weekend shifts per month.
  • Summer Holiday Coverage: Must work at least one of the following major Summer Holidays:
  • Memorial Day
  • Independence Day (July 4th)
  • Labor Day
  • Winter Holiday Coverage: Must work at least one of the following major Winter Holidays:
  • Thanksgiving Day
  • Christmas Day
  • New Year's Day
