Base pay range

$48.00/hr - $55.00/hr

Registered Nurse (RN) – Home Health

Location: Murrieta, CA

Schedule: 5 days/week with rotating weekend coverage (up to 3 weekend days/month, with weekday off in exchange)

Compensation: $48–$55 per hour (depending on experience)

Travel Required: Yes (valid driver’s license, insurance, and reliable vehicle required)

Position Summary

As a Registered Nurse (RN) in Home Health, you will be responsible for evaluating, planning, and implementing nursing services to individuals in their homes. You will lead patient care efforts, guide care plans, coordinate with interdisciplinary teams, and support patients and families through their health journeys.

Key Responsibilities

  • Perform initial patient assessments and gather comprehensive health histories
  • Develop and revise individualized care plans based on changing needs and diagnoses
  • Administer prescribed treatments and medications
  • Provide health education and guidance to patients and families
  • Identify and implement appropriate discharge planning procedures
  • Deliver preventive, therapeutic, and rehabilitative nursing care per physician orders
  • Maintain accurate and timely clinical documentation in compliance with agency standards

Required Qualifications

  • Active California Registered Nurse (RN) License
  • Graduate of an accredited school of nursing (Associate’s or Bachelor’s)
  • BLS CPR Certification (hands-on)
  • Minimum 1 year of recent acute care experience (hospital or home health setting)
  • Valid driver's license, personal auto insurance, and reliable transportation
  • Demonstrated competence in nursing assessments, care planning, and patient safety
  • Excellent verbal and written communication skills

Preferred Qualifications

  • Bachelor’s Degree in Nursing (BSN)
  • Previous experience in a home health or community-based care environment

Physical Requirements

  • Frequent walking, standing, and sitting (2–3 hours each per 8-hour shift)
  • Ability to lift/carry up to 50 lbs frequently and 100 lbs occasionally
  • Ability to push/pull loads ranging from 0 to 100+ lbs
  • Physical agility to kneel, bend, reach, twist, climb stairs, and occasionally walk on uneven terrain
  • Capacity to perform patient handling and environmental assessments across various home settings
