Job summary

Entyre Care seeks an RN Supervisor to lead in-home assessments, develop plans of care, and train caregivers. You will serve as the clinical backbone, reporting to the Program Director and shaping how the agency delivers care in clients’ homes.

You’ll bridge families, caregivers, managed care, and internal teams to ensure compliant, seamless care delivery, with a strong focus on autonomy and field leadership.

Qualifications

• Experience in home care, personal care, or community-based nursing preferred.
• Comfort working independently in clients' homes across Milwaukee County.
• Valid driver’s license and reliable transportation.
• Strong documentation habits to protect clients and the agency

Responsibilities

• Conduct in-home client assessments and evaluate needs against nursing standards.
• Develop individualized plans of care following each home visit.
• Obtain and maintain signed physician orders; renew every three months or on a change in condition.
• Review and update plans every 60 days or when a client’s condition changes.
• Provide written instruction and hands-on training for personal care workers.
• Delegate clinical tasks, evaluate competency, and retrain where gaps appear.
• Support new caregivers through orientation and ongoing field mentorship.
• Conduct scheduled and unannounced supervisory home visits at least every 60 days.
• Serve on the Quality Assessment and Assurance Committee and contribute to operational planning.
• Coordinate referrals to additional services as client needs evolve

Entyre Care helps elders and peoples with disabilities stay and health at home with trusted caregivers. As an RN Supervisor, you are the clinical backbone of that model — you assess clients in their homes, build their plans of care, and train and mentor their caregivers.

This is hands-on, in-the-field nursing with real autonomy. You will report to the Program Director and help shape how a growing agency delivers care.

This role serves as the operational bridge between members, family caregivers, managed care organizations, and Entyre’s internal teams to ensure a seamless and compliant care experience.

What You’ll Do:

• Obtain and maintain signed physician orders; renew every three months or on a change in condition
• Review and update plans every 60 days, or whenever a client's condition changes
• Provide written instruction and hands-on training so personal care workers demonstrate skill mastery before working independently
• Delegate clinical tasks, evaluate competency, and retrain where gaps appear
• Support new caregivers through orientation and offer ongoing mentorship in the field
• Conduct scheduled and unannounced supervisory home visits at least every 60 days — observing care delivery, auditing daily records, and documenting any changes needed
• Serve on the Quality Assessment and Assurance Committee and contribute to operational planning
• Coordinate referrals to additional services as client needs evolve
