Job summary

Entyre Care is seeking a detail-oriented Care Manager to perform home visits, train caregivers, and support client care plans. You will monitor health, ensure quality care, and maintain regulatory compliance while coordinating with physicians and resources.

The role offers a hybrid, full-time arrangement with meaningful work that improves families’ lives and supports caregivers in the home setting.

Qualifications

• 1+ year of in-home healthcare experience required.
• RN license is required.
• Experience in case management, care planning and assessment in home care is preferred.

Responsibilities

• Conduct home visits and create personalized care plans for clients and caregivers.
• Train and support caregivers to meet client needs.
• Monitor clients’ health and report to primary care physicians; coordinate with community resources.
• Facilitate ongoing care transitions and ensure compliance with regulations and standards.
• Engage in community outreach to enroll new caregivers.

Position Overview:

We are seeking a detail-oriented and organized Care Manager to conduct home visits, train, and support our caregivers, ensuring they can meet their loved ones’ needs. In this role, you will monitor and report on clients’ health, support personalized care plans, ensure ongoing quality care, and maintain compliance with all relevant regulations and standards. Key Responsibilities:

• Home visits: Conduct home visits to assess and create personalized care plans for clients and their caregivers.
• Training & Support: Train and support caregivers, ensuring they can meet their loved ones' needs.
• Reporting & Coordination: Monitor and report on clients’ health, coordinating with their primary care physicians, community resources, and more.
• Ongoing care: Facilitate smooth transitions and ongoing care for clients.
• Compliance: Maintain compliance with all relevant regulations and standards.
• Community awareness: Participate in community events to spread awareness and enroll new caregivers.
• Role Model: Embody Entyre Care’s mission, values, and culture, acting as a role model for the team.

What You Will Bring:

• RN license required.
• Bachelor’s degree appreciated not required; preferably in social work, human services, or a related field.
• Ability to travel (by automobile) within assigned geographic area; valid driver’s license and automobile insurance is required.
• At least one (1) year of experience in-home healthcare.
• Experience in case management, care planning and assessment for cases including but not limited to eldercare, people with disabilities, and other complex medical needs in a home care setting.
• Experience supervising caregivers preferred
• Proficiency in common software and web-based applications (e.g. MS Office, case management systems, PCP order forms etc).
