Job summary

Entyre Care seeks a detail-oriented Care Manager to conduct home visits, train and support caregivers, and ensure high-quality, compliant care for clients at home. You will monitor client health, support care plans, and coordinate with physicians and community resources to deliver exceptional service.

The role requires RN licensure, travel within the designated area, and 1 year of in-home healthcare experience.

Qualifications

• RN license required.
• Bachelor’s degree appreciated not required; preferably in social work, human services, or a related field.
• Ability to travel (by automobile) within assigned geographic area; valid driver’s license and automobile insurance is required.
• At least one (1) year of experience in-home healthcare.
• Experience in case management, care planning and assessment for cases including but not limited to eldercare, people with disabilities, and other complex medical needs in a home care setting.
• Experience supervising caregivers preferred
• Proficiency in common software and web-based applications (e.g. MS Office, case management systems, PCP order forms etc).

• Home visits: Conduct home visits to assess and create personalized care plans for clients and their caregivers.
• Training & Support: Train and support caregivers, ensuring they can meet their loved ones' needs.
• Reporting & Coordination: Monitor and report on clients’ health, coordinating with their primary care physicians, community resources, and more.
• Ongoing care: Facilitate smooth transitions and ongoing care for clients.
• Compliance: Maintain compliance with all relevant regulations and standards.
• Community awareness: Participate in community events to spread awareness and enroll new caregivers.
