ComForCare Home Care is seeking a Registered Nurse to conduct comprehensive client assessments, create personalized care plans, and implement interventions to support clients living at home in Massachusetts. You will coordinate with the health team, maintain detailed records, and administer medications as needed.

The role emphasizes safety, infection control, and visiting clients in their homes or facilities, with a focus on improving client outcomes and quality of life.

Qualifications

• Graduate of an accredited school of professional nursing.
• Current RN license in Massachusetts.
• CPR certification.
• Knowledge of safety and infection control practices.
• Comfort visiting clients at home or in facilities.

Responsibilities

• Perform comprehensive client assessments (physical, psychosocial, environmental).
• Develop individualized care plans based on data and evidence.
• Provide interventions to achieve realistic outcomes.
• Report changes to update care plans.
• Administer medications as needed.

What we are looking for

• Graduate of an accredited school of professional nursing
• A passion to serve and help others live their best lives possible
• Current license as a Registered Nurse (RN) in the state of Mass.
• CPR certification
• Demonstrated knowledge of safety and infection control practices
• Must feel comfortable seeing our clients in their homes and or facilities

What you will be doing

• Performing comprehensive assessments of client status, including physical, psychosocial, and environmental parameters
• Developing an individualized plan of care for each client that incorporates analysis of data and current scientific findings
• Providing interventions with a focus on achieving realistic client outcomes
• Reporting changes to update the plan of care to reflect progress towards goals and outcomes
• Administering medications as needed
• Maintaining client records showing systematic assessments, planning interventions, and evaluations
• Initiating referrals as needed to other health team members

Salary Range

• $60-120 per visit
