Job summary

ComForCare Home Care is seeking a Registered Nurse to conduct client assessments, develop and implement personalized care plans, and coordinate referrals. You will administer medications, maintain comprehensive records, and ensure safety and high-quality home care for clients in Dallas, TX.

Qualifications

• Graduate of accredited school of professional nursing.
• Current license as a Registered Nurse (RN) in TX of practice.
• CPR certification.
• Demonstrated knowledge of safety and infection control practices.

Responsibilities

• Perform comprehensive assessments of client status (physical, psychosocial, environmental).
• Develop individualized care plans based on data and current findings.
• Provide interventions focused on achieving realistic client outcomes.
• Report changes to update the plan of care reflecting progress towards goals.
• Administer medications as needed.
• Maintain detailed client records of assessments, plans, and evaluations.
• Initiate referrals to other health team members as required.

What we are looking for:
