Qualifications

• Develop care plans: Create personalized plans prioritizing patient needs and goals.
• Deliver patient care: Provide evidence-based care, educate patients and families, document interventions.
• Evaluate outcomes: Monitor responses and adjust care based on outcomes and evidence.
• Drive improvement: Support Performance Improvement initiatives with the Clinical Manager.
• Promote High Reliability: Apply High Reliability principles for patient care and operations.
• Lead and mentor: Guide peers and team members while managing patient care.
• Document care: Accurately document nursing process and education in the EMR.
