Job summary

Baptist Health Arkansas is seeking an RN Case Manager for Home Health/Hospice to provide, manage and coordinate care for an assigned caseload of patients.

Responsibilities include coordinating services, collaborating with care teams, and ensuring quality patient outcomes. Education: graduation from an approved nursing program; must hold an Arkansas RN license and a valid Arkansas driver's license. Home Health/Hospice experience preferred.
