Job summary

Baptist Health Arkansas is seeking an RN Case Manager to provide, manage, and coordinate care for an assigned caseload of patients in home health and hospice settings.

The ideal candidate has graduation from an approved nursing program, an Arkansas RN license, and a valid Arkansas driver's license; 1 year of nursing experience in home health or hospice is preferred. This role requires strong communication and care coordination with the care team.
