Job summary

Palms-Home-Care in Florida seeks a dedicated RN for home health. You will assess patients, develop care plans, coordinate with the care team, and educate caregivers to improve health outcomes. Travel to patient homes is required.

FT and PRN positions available. Florida RN license and at least 1 year of home health experience are required. Strong communication and organizational skills are essential.

• Communicate effectively with the clinical manager and interdisciplinary team regarding patient status and care updates.
• Educate patients and caregivers to promote independence and improve health outcomes, including training in self-care techniques.
• Coordinate with Physicians, Physical Therapists, Occupational Therapists and other individuals in the patient’s care plan.
• Supervise and support LPNs to ensure high standards of care.
• Perform additional duties as needed to support patients and team operations.
