Job summary

Angels Care Home Health in Jasper, IN is seeking a Registered Nurse (RN) Case Manager to deliver high-quality patient care in the home setting.

You will conduct admission visits, assess eligibility for home health services, and develop the care plan in collaboration with other medical disciplines.

Qualifications

• Graduate of an accredited diploma, associate or baccalaureate School of Nursing.
• Current state license as a Registered Nurse.
• Current state Driver’s License.
• One-year experience as a Registered Nurse, two preferred.
• Proof of CPR and Hepatitis consent/declination.
• Reliable transportation with valid auto liability insurance.
