Job summary

Angels Care Home Health is seeking a Registered Nurse Case Manager in Glendale, AZ to coordinate home care for patients. You will perform admission visits, assess eligibility for home health services, and develop individualized care plans in collaboration with multidisciplinary teams.

Qualifications

• Graduate of an accredited diploma, Associate or Baccalaureate School of Nursing.
• Current state license as a Registered Nurse.
• Current state Driver’s License.
• One-year’s experience as a Registered Nurse, two preferred.
• Proof of current CPR and Hepatitis consent/declination.
• Reliable transportation with valid auto liability insurance.

Responsibilities

• Coordinate all aspects of the patient’s home care.
• Interact with other medical disciplines as needed and provide professional care.
