Job summary

Angels Care Home Health in Surprise, AZ is seeking a Registered Nurse (RN) Case Manager to coordinate all aspects of the patient’s home care. You will work with medical teams and develop plans of care, including admission visits.

The ideal candidate is a graduate of an approved nursing program, holds a current RN license, has a valid driver’s license and reliable transportation, and values professional growth through training and CEUs.

Qualifications

• Graduate of an accredited nursing program (Diploma/ASN/BSN).
• Current state license as a Registered Nurse.
• Current state driver’s license.
• One year of RN experience preferred (two years recommended).
• CPR certification and hepatitis consent/declination.
• Reliable transportation with valid auto liability insurance.

Responsibilities

• Coordinate all aspects of the patient’s home care.
• Interact with other medical disciplines as needed and provide professional care.
