Job summary

Angels Care Hospice in Texas is seeking a Registered Nurse Case Manager to coordinate all aspects of patient home care. You will conduct admission visits, determine eligibility for home health services, and develop the plan of care in collaboration with the patient and medical team.

The ideal candidate holds an accredited nursing diploma/degree, an active RN license, a valid driver’s license, CPR certification, and at least one year of RN experience (two preferred).

Qualifications

• Active RN license in the state of Texas.
• CPR certification required.
• Reliable transportation with auto liability insurance.

Responsibilities

• Coordinate all aspects of the patient’s home care as the RN Case Manager.
• Perform admission visits and assess eligibility for home health services.
• Develop and implement the plan of care in collaboration with the patient and care team.

About the position:

The Registered Nurse (RN) Case Manager coordinates all aspects of the patient’s home care. You will interact with other medical disciplines as needed & provide professional care to the patient. You will also perform the admission visit to the patient in the home, determine the patient’s eligibility for home care services, & develop the plan of care to be followed.

Qualifications:

• Graduate of an accredited diploma, Associate or Baccalaureate School of Nursing.
• Current state license as a Registered Nurse.
• Current state Driver’s License.
• One-year’s experience as a Registered Nurse, two preferred.
• Proof of current CPR and Hepatitis consent/declination.
• Reliable transportation with valid and current auto liability insurance.
