Job summary

Discovery at Home in Houston is seeking a Registered Nurse for home health care. The role requires an active nursing license and a BSN or ADN, with at least one year of home health experience preferred. You will assess needs, create care plans, administer treatments, and educate patients and families.

The RN will collaborate with an interdisciplinary team, maintain records, and respond to emergencies. A valid driver's license and CPR certification are required to support community-based care.

Qualifications

• Must be licensed or registered by the applicable state without restrictions.
• Bachelor’s degree in nursing (BSN) or Associate degree in Nursing (ADN) from an accredited nursing program.

Responsibilities

• Provide comprehensive nursing care to patients, adhering to professional nursing standards, policies, and procedures.
• Assess patient needs, develop, and implement individualized care plans to ensure optimal patient outcomes.
• Administer medications, treatments, and therapies as prescribed by healthcare providers.
• Monitor and document patient progress, including vital signs, symptoms, and changes in medical conditions.
• Collaborate with interdisciplinary healthcare team members to coordinate and provide continuity of care.
• Educate patients and their families about treatment plans, medications, and self-care techniques.
• Respond to emergency situations and perform life-saving interventions promptly and accurately.
• Maintain accurate and up-to-date medical records of patients' health history, treatment plans, and progress.
• Adhere to infection control protocols and ensure a safe and healthy environment for patients and staff.
• Stay current with advances in nursing practices and attend training programs and continuing education to enhance professional development.
• Build positive working relationships with community leadership, wellness teams, residents, and families.

• Serve as a visible clinical representative of Discovery At Home within assigned IL/AL communities.
• Educate residents, families, and community staff about available home health and supportive services when appropriate.
• Identify potential service opportunities and communicate them to the appropriate Discovery At Home intake or business-development team.
• Participate occasionally in community events, health fairs, educational programs, or other outreach activities.
• Support community relationship development while maintaining the RN's primary focus on quality patient care.

Qualifications:

• Must be licensed or registered by the applicable state without restrictions.
• Bachelor’s degree in nursing (BSN) or Associate degree in Nursing (ADN) from an accredited nursing program.
• One year of home health experience required
• Must have knowledge of Medicare and Medicaid guidelines

• Must have a valid driver’s license and reliable transportation.
• Must have a current CPR certification.
• Excellent verbal and written communication skills.
• Basic computer skill proficiency. Ability to use Microsoft Windows.
