Job summary

Amedisys Home Health is seeking a Registered Nurse Case Manager in Homewood, AL, to join a leading home health and hospice team. The role focuses on patient assessments, care planning, and coordination with the care team to promote healing at home.

The position requires RN licensure, CPR, and at least 1 year of clinical RN experience. You will supervise LPNs and HHAs, complete documentation, and participate in ongoing clinical development and on-call services.

Qualifications

• One (1+) year of clinical RN experience; higher experience preferred.
• Current RN license for the state(s) assigned.
• Current CPR certification.

Responsibilities

• Performs patient assessments and collaborates with the care team to develop and implement a plan of care.
• Makes referrals to other disciplines as indicated by patient needs.
• Promotes patient health and independence through teaching and rehabilitative measures and self-care.
• Supervises LPNs and HHAs.
• Completes timely and accurate documentation.
• Communicates patient progress to the clinical manager and care team.

Attractive pay

$73,000.00 to $80,000.00 Salary for Orientation and will then convert to a per point pay method.

Responsibilities

• Performs patient assessments and collaborates with the care team to develop and implement a plan of care.
• Makes referrals to other disciplines as indicated by the patient's identified needs or documents rationale for not doing so.
• Promotes patient health and independence through teaching and appropriate rehabilitative measures, assisting patients in learning appropriate self-care techniques.
• Supervises LPNs and HHAs.
• Completes documentation timely and accurately.
• Regularly communicate patient progress to the clinical manager and care team.
• Plans and provides staff education.
• Performs on-call responsibilities and on-call services to patients/families as assigned.
• Participates in clinical development and continuing education programs.
• Other duties as assigned.

Qualifications

• One (1+) year of clinical experience as a Registered Nurse (RN). If less than 1 year clinical experience as a RN, candidate must be approved by VP Clinical.*
• Current RN license, specific to the state(s) you are assigned to work.
• Current CPR certification.
• Valid driver's license, reliable transportation and liability insurance.
• Note - If less than 6 months clinical experience as a RN, candidate must participate in RN Intern program.
