Job summary

Angels Care Home Health in Texas is seeking an experienced Registered Nurse Case Manager to coordinate all aspects of a patient’s home care. You will perform admission visits, assess eligibility, and develop the plan of care in collaboration with the patient and family.

The role requires Texas RN license, valid driver’s license, CPR, hepatitis consent/declination, and reliable transportation. One year of RN experience; home health preferred.

Qualifications

• Graduate of an accredited nursing program.
• Current Texas RN license.
