Job summary

Angels Care Home Health is seeking a Registered Nurse Case Manager to coordinate home health care for patients in the Bay City area. You will interact with various medical disciplines and provide professional care, including admitting patients to home care and shaping their care plans.

The role requires RN licensure, a valid driver’s license, CPR, and reliable transportation. We offer competitive pay, vehicle allowance, PTO, and opportunities for professional development.

Qualifications

• Graduate of an accredited nursing program (Diploma, ADN, or BSN).
• Current state RN license.
• Current state Driver's License.
• One year of RN experience; two years preferred.
• CPR certification and Hepatitis consent/declination.
• Reliable transportation with valid auto liability insurance.

Responsibilities

• Coordinate all aspects of the patient’s home care as the RN Case Manager.
• Collaborate with other medical disciplines and provide professional patient care.
• Perform admission visit to the patient’s home and determine eligibility for home care services.
• Develop the plan of care to be followed.
