Job summary

Angels Care Home Health is seeking an RN Case Manager to coordinate patient home care, conduct admission visits, and determine eligibility for home services. You will work with diverse medical teams to implement and monitor the plan of care in patients' homes.

Qualifications include an RN license, nursing degree, valid driver’s license, and 1 year of RN experience (2 preferred). Benefits include health, PTO, and professional development opportunities.

Qualifications

• Graduate of an accredited nursing program (Diploma/ASN/BSN).
• Current state RN license in good standing.
• Valid driver’s license and reliable transportation.
• CPR certification and Hepatitis consent/declination.
• One year RN experience; two preferred.

Responsibilities

• Coordinate all aspects of the patient’s home care.
• Interact with other medical disciplines as needed.
• Perform admission visit to the home and assess eligibility.
• Develop and implement the plan of care for home services.
• Monitor and adjust care plan in collaboration with the team.
