Job summary

BAYADA Home Health Care is seeking a part-time Registered Nurse (RN) to perform home health visits in New Jersey. The RN will follow care plans, administer medications, and monitor patient conditions within a multi-disciplinary team.

Requirements include NJ RN license, 1 year of clinical experience, and willingness to travel. EMR experience is preferred and opportunities for professional growth are offered.

Qualifications

• Current NJ RN license.
• Minimum 1 year of recent clinical (medical/surgical) experience.
• Graduation from an accredited nursing program.
• Ability to travel to cases as assigned.
• Strong interpersonal and time-management skills.
• Experience with electronic medical records (EMR) preferred.

Responsibilities

• Follow designated care plans in collaboration with the patient and care team.
• Perform home visits to clients in assigned geographic territories.
• Administer medications, provide wound care, and carry out treatments per orders.
• Monitor patient conditions and report changes to supervisor.
• Follow up on, execute, and document doctors’ orders accurately.
• Conduct client assessments as needed and participate in case management.
• Document observations and interventions using the tablet EMR.
• Contribute to coordinating care across multidisciplinary teams.

Registered Nurse (RN) Responsibilities:

• Follow a designated care plan in accordance with patient’s needs
• Makehome visits to clients in designated geographic territories
• Perform assigned duties, including administration of medication, wound care, treatments, and procedures
• Monitor clients’ conditions; reporting changes to Clinical or Client Services Manager
• Follow up with, execute, and properly document doctors’ orders
• Perform client assessments as necessary
• Case management and coordination
• Accurately document observations, interventions, and evaluations pertaining to client care management and services provided, utilizing a state-of-the-art touch pad tablet

Qualifications for a Registered Nurse (RN):

• A current license as a Registered Nurse in NJ .
• A minimum of one year of recent, verifiable clinical (medical/surgical) experience.
• Prior home care experience strongly preferred, but not required.
• Graduation from an accredited and approved nursing program, as indicated by school transcript or diploma.
• Ability to work independently and manage time effectively.
• Strong interpersonal skills.
• Solid computer skills; prior experience with electronic medical records (EMR) preferred.
• Ability to travel to cases as assigned.

Additional Information:

Base Pay, depending upon qualifications and subject to negotiation:
$50.00-$57.00 per point
