Job summary

BAYADA Home Health Care is seeking a Registered Nurse (RN) for a full-time role delivering one-on-one skilled nursing visits to adult and geriatric clients in Senior Living communities across Somerset, Middlesex, Monmouth and Union Counties, NJ. You will work with an interdisciplinary team to provide high-quality care in the home and community settings.

Responsibilities include assessments, care planning, medication administration, wound care, and documentation.

Qualifications

• Active New Jersey RN license
• Minimum 1 year of recent clinical nursing experience
• Home health or senior living experience preferred but not required
• Ability to travel throughout Somerset, Middlesex, Monmouth and Union Counties
• Graduate of an accredited nursing program
• EMR experience preferred

• Provide skilled nursing visits to clients in designated senior living communities and territories.
• Complete comprehensive nursing assessments and develop individualized plans of care.
• Administer medications, perform wound care and physician-ordered treatments.
• Monitor client conditions and report changes to Clinical Manager or Client Services Manager.
• Document physician orders accurately and timely.
• Coordinate care with physicians, therapists and interdisciplinary team.
• Educate clients and caregivers to promote health, safety and independence.
• Complete clinical documentation in BAYADA EMR system.

Make an Impact as a Registered Nurse (RN) at BAYADA Home Health Care

BAYADA Home Health Care is seeking a Registered Nurse (RN) for a full-time opportunity with our new Central NJ Senior Living team, a Medicare-certified home health office.

This position provides one-on-one skilled nursing care to adult and geriatric clients living in assisted living, independent living, senior congregate living, 55+ communities, and intermediate care facilities serving individuals with intellectual disabilities throughout Somerset, Middlesex, Monmouth, and Union Counties, NJ.

As a BAYADA Registered Nurse, you'll have the opportunity to build meaningful relationships with your clients while providing high-quality clinical care in the place they call home. You'll work independently while being supported by an experienced interdisciplinary team that values your clinical expertise.

This position may also include seeing clients in traditional home health community settings.

How You’ll Make an Impact:

• Provide skilled nursing visits to clients throughout designated senior living communities and territories.
• Complete comprehensive nursing assessments and develop individualized plans of care based on client needs.
• Administer medications, provide wound care, and perform physician-ordered treatments and procedures.
• Monitor client conditions and communicate changes to the Clinical Manager or Client Services Manager.
• Follow, execute, and accurately document physician orders.
• Provide case management and coordinate care with physicians, therapists, and other members of the interdisciplinary team.
• Educate clients and caregivers to promote health, safety, and independence.
• Complete timely and accurate clinical documentation using BAYADA's electronic medical record (EMR) system.

Qualifications:

• Current New Jersey Registered Nurse (RN) license.
• Minimum of one year of recent, verifiable clinical nursing experience.
• Medical-surgical experience preferred.
• Graduate of an accredited and approved nursing program.
• Home health or senior living experience preferred, but not required.
• Ability to work independently and effectively manage time.
• Strong communication, interpersonal, organizational, and clinical assessment skills.
• Experience with electronic medical records (EMR) preferred.
• Reliable transportation and ability to travel throughout Somerset, Middlesex, Monmouth, and Union Counties.

Additional Information:

Base Pay, depending upon qualifications, experience, and subject to negotiation:

$65.00 - $75.00 per point ($101,400 - $117,000)
