Job summary

Alpha Home Health and Hospice seeks a skilled MDS Nurse RN to coordinate and manage the MDS/RAI process in a skilled nursing and rehabilitation setting.

You will complete, audit, and coordinate MDS assessments, collaborate with an interdisciplinary team, and ensure accuracy and regulatory compliance to support optimal resident care and reimbursement processes.

Qualifications

• Current RN licensure.
• Strong knowledge of MDS/RAI processes, diagnosis coding, and care area assessments.
• Excellent attention to detail, documentation accuracy, and compliance mindset.
