Job summary

Bristol Hospice Detroit, MI is seeking an experienced RN Case Manager who will deliver compassionate, patient- and family-centered end-of-life care across settings. You will develop individualized care plans, monitor clinical status, and coordinate with the care team to ensure comfort, dignity and timely interventions.

Responsibilities include documenting vitals, administering physician-ordered treatments, educating families, and navigating the care journey with empathy.

Qualifications

• Must have a valid RN license in good standing in the state of work (Michigan).
• CPR certification (current).
• Strong observational and communication skills—written and verbal.
• Valid driver’s license and reliable transportation.

• Develop and implement individualized care plans based on medical history and current needs.
• Provide one-on-one support to patients and families ensuring comfort and dignity.
• Conduct assessments of physical, emotional, and spiritual needs throughout the care journey.
• Monitor and document vital signs including temperature, pulse, respiration, and blood pressure.
• Administer physician-ordered medications and treatments with skill and empathy.
• Educate patients and families on care techniques and symptom management.

• Deliver attentive, one-on-one support to patients and their families, ensuring comfort, dignity, and understanding
• Conduct thorough assessments of physical, emotional, and spiritual needs throughout the care journey
• Accurately monitor and document vital signs including temperature, pulse, respiration, and blood pressure
• Administer physician-ordered medications and treatments with skill and empathy
• Educate patients and their families on care techniques and symptom management
• Track and communicate changes in patient condition, ensuring timely and effective responses
• Keep detailed and up-to-date clinical notes to reflect patient progress and quality of care provided
• Collaborate with interdisciplinary teams to ensure comprehensive care delivery
• And contribute to other supportive duties as part of a compassionate, mission-driven team
• And additional duties as assigned

What You’ll Bring to The Team:
